By clicking on the "Submit" button below, I the parent/guardian of the child/children listed below, hereby release the Diocese of Pueblo, Immaculate Heart of Mary Catholic Church, it's staff, and the adult volunteers, of the reponsibility regarding possible injury, accident, sickness, etc.,  that might occur during the event.  In understand that the organizers are taking all reasonable precautions to make this a safe and enjoyable experience.  I also understand that if the child/children is/are ill, they will not be allowed to attend VBS that day.  Immaculate Heart of Mary will make every effort to reimburse the pro-rated registration fee.
